Each Client that is seen with Radiant Souls Bodywork is required to fill out paperwork prior to your first service, which will be sent to you through our scheduling app service. Please take time to fill out prior to the day of service to allow us time to look over your forms and to be better prepared for your session.

The Forms required before service are:

• Intake Form
• Radiant Souls Bodywork Therapy Agreement
• Informed Consent
• Minor Consent Form, if applicable

DAY OF SERVICE EXPECTATIONS

At time of service, please arrive 5 minutes prior to allow time for us to discuss your needs for the session and anything else that may arise. If possible, take off any jewelry prior to service. 

CANCELLATION POLICY

Any cancellations needed are required with a 24 hour notice prior to appointment.  If 24 hours is not given to the therapist or the client is a no-show, a $25 charge will be expected from the client. In the case of a cancellation without 24 hour notice or a no-show, Radiant Souls will notify the client by email prior to taking payment due, from the credit card used at the time of scheduling.  If the client wishes to use an alternative form of payment, they are expected to notify Radiant Souls immediately. If Radiant Souls is not notified of an alternative form of payment within 2 hours of notice by email, Radiant Souls will take the $25 fee due from the credit card on file. Exceptions to this policy are sudden illness, accidents, emergency and weather related issues and is at the discretion of the therapist with Radiant Souls.

ILLNESS/LATENESS

If the client is experiencing cold or flu-like symptoms, has had a recent injury within 72 hours of appointment or is under a physician's care for a serious health issue, the client is responsible to notify Radiant Souls as soon as possible so that appointment may be rescheduled. Clients are not to show up to appointments with fever or any illness that could be contagious.
